各位老師您好：
誠摯邀請您推薦本學期擔任教學助教（TA）之學生申請「114-2學期優良教學助教獎勵金」。中心將擇優15位優良TA頒發每人三千元獎勵金。
【申請資格】
1. 擔任114-2學期之教學助教
2. 持有有效之TA證書或完訓2+2TA課程之學習護照
3. 經授課教師同意，配合教學卓越中心於課堂中施測《TA滿意度調查問卷》
【申請期間】
115年5月18日（星期一）至5月22日17:00前（星期五）

1. 授課教師推薦表與對TA滿意度評分表
2. 優良TA申請表與自評表

Application for the TA Excellence Award (Semester 114-2)
Dear Faculty Members,
We sincerely invite course instructors to recommend their teaching assistants (TAs) for the TA Excellence Award for the 114-2 semester. The Center will select 15 outstanding TAs, each of whom will receive a NT$3,000 award.
Eligibility
• Currently serving as a TA in the 114-2 semester
• Holding a valid TA Certificate or a Learning Passport showing completion of the 2+2 TA training
• Having obtained the course instructor’s consent for the Center for Teaching Excellence to administer the TA Satisfaction Survey during class
Application Period
From Monday, May 18 to Friday, May 22, 2026, by 5:00 PM
Required Documents
• Instructor Recommendation Form and Instructor’s TA Satisfaction Evaluation Form
• TA Excellence Award Application Form and TA Self-Evaluation Form
Please submit all required documents to Mr. You at the Center for Teaching Excellence (Administration Building, Room 202). Late submissions will not be accepted. If you have any questions, please feel free to contact the Center for Teaching Excellence.
