各位老師您好：

誠摯邀請您推薦您課堂的教學助教（TA）申請「113-2學期優良教學助教獎勵金」💰。

申請資格

1. 擔任113-2學期之教學助教
2. 持有有效之TA證書或學習護照
3. 經授課教師同意，配合教學卓越中心於課堂中施測《TA滿意度調查問卷》

申請期間

114年5月12日（星期一）至5月16日（星期五）

申請資料

1. 授課教師推薦表與對TA滿意度評分表
2. 優良TA申請表與自評表

以上申請資料送至教學卓越中心 游先生（行政大樓411室），逾期恕不受理，如有任何疑問，歡迎與教學卓越中心聯繫。

Dear Faculty Members,
 

Course instructors are invited to recommend their teaching assistants (TAs) for the TA Excellence Award for the 113-2 semester.

To be eligible, the TA must:

• Be currently serving as a TA in the 113-2 semester
• Hold a valid TA certificate or Learning Passport
• Have the course instructor’s consent to allow the Center for Teaching Excellence to conduct the TA Satisfaction Survey in class

Application Period:
Monday, May 12 – Friday, May 16

Please submit the following documents to Mr. You at the Center for Teaching Excellence (Administration Building, Room 411):

• Instructor Recommendation Form
• TA Excellence Award Application Form
