標題： 【113-1教卓中心】TA證書與優良TA獎勵金申請 / Application for TA certificate & TA Excellence Awards

Dear TA們~

有關113-1學期TA證書與TA獎勵金申請方式，請參閱以下說明。

【TA證書申請】

完成 2必修+ 2選修 = 4門課程的同學，即日起至教學卓越中心網站申請證書，並上傳符合2+2課程之學習護照以示證明。因證書採用批次印製，114/1/5 (日)之前申請同學於114/1/10〈五〉起至中心領取TA證書；114/1/6(一)之後申請者請於下學期開學後領取。若已有證書且本學期擔任TA者，須選擇一門實體或線上課程進修回流。

申請證書注意事項

1. 跨學期修課可列入計算
2. 其中實體至少2門
3. 修讀學術倫理證明與e等公務員平台修讀性別課程可抵免線上各一門選修
4. 相關附件證明請自行透過ppt或word方式將檔案合併成一個檔案上傳

【優良TA獎勵金申請】

欲申請優良TA的同學，若您是擔任本學期〈113-1〉的教學助理，並持有TA證書，且授課教師同意卓越中心施測《TA滿意度調查問卷》，申請人請於12/2〈一〉-12/6〈五〉17：00前將優良TA申請表送至教學卓越中心(行政大樓411)游先生，逾期不受理，趕緊把握機會，我們將擇優選拔15名優良TA〈可獲優良TA獎狀一式與三千元獎勵金〉！！若尚未領取TA證書，可用學習護照代替證明。

【113-1 Application for TA certificate & TA Excellence Awards】

By completing two compulsory courses and two elective courses this semester, TAs are eligible to apply for the TA certificate. Applications submitted before Sunday, January 5, 2025, will be processed, and certificates will be available starting Friday, January 10, 2025.

Submission Location: Center for Teaching Excellence (Administration Building, Room 411)

If you are a current TA with a TA certificate (or Learning Passport), and your instructor agrees to let us conduct a student survey in your class, you are eligible to apply for the TA Excellence Awards. The application period is from Monday, December 2, to 5:00 PM on Friday, December 6. Please submit your application to the Center for Teaching Excellence (Administration Building, Room 411).
