Dear TA們
有關112-2學期TA證書與TA獎勵金申請方式，請參閱以下說明。
【TA證書申請】
完成 2必修+ 2選修 = 4門課程的同學，即日起至 https://ndhucte.ndhu.edu.tw/p/423-1008-2370.php申請證書，並上傳符合2+2課程之學習護照以示證明。因證書採用批次印製，113/5/31 (五)之前申請的同學於113/6/11〈二〉起至中心領取TA證書；113/6/1(六)之後的申請者請於113/7/8(一)後或下學期開學後領取。若已有證書且本學期擔任TA者，須選擇一門實體或線上課程進修回流。
申請證書注意事項：
- 跨學期修課可列入計算
- 其中實體至少2門 (可含直播)
- 修讀學術倫理證明可抵免1門線上必或選修課程，e等公務員平台修讀性別課程可抵免1門線上選修
- 請自行透過ppt或word將相關附件、證明合併成一個檔案上傳。
【優良TA獎勵金申請】
申請資格：
1. 擔任本學期〈112-2〉的教學助理，並持有TA證書。
2. 授課教師同意卓越中心施測《TA滿意度調查問卷》。
欲申請優良TA的同學請於113/5/13〈一〉～113/5/17〈五〉將優良TA申請表掃描成電子檔(pdf)、寄至承辦人信箱，逾期不受理，趕緊把握機會。
我們將擇優選拔15名優良TA〈可獲優良TA獎狀一式與三千元獎勵金〉！！若同學尚未領取TA證書，可先用學習護照代替證明。

附註：
1.若授課教師因遠距上課無法簽名，可提供教師電子簽名或請授課教師寄信至承辦人信箱，以作為同意TA申請優良TA獎勵金的佐證。
2.中心將於113/5/20(一)(112-2學期第十四周)寄送相關問卷表單的連結給教師與TA，請課堂學生線上填寫，填答時間至這學期第十七周截止。
【112-2 Application for TA certificate & TA Excellence Awards】
With completing 2 compulsory courses and 2 elective courses this semester, TAs are qualified to apply for the TA certificate. If you apply for TA certificate after Jun. 1(Sat), and then you can receive the certificate after Jul, 8(Monday) or next semester school open.
Place: Center for Teaching Excellence (Administration Building, Room 411)

If you are a current TA with the TA certificate (or learning passport), you may apply for the TA Excellence Awards from May, 13(Mon) to May, 17(Fri) and send the application to hyttepd@gms.ndhu.edu.tw.*

The center will send the relevant questionnaire links to teachers and TAs on the 14th week(May, 20). Please have the students fill out the questionnaire online. The deadline for completion is by the end of the 17th week.
