各位同學及教職員，大家好：

因應全球疫情快速升高，教育部於109年3月16日通報各校(詳見附件一)，請境外返臺之教職員工生健康管理配合事項，配合健康管理措施如下:
一、本校教職員工生，如因個人行程出遊返國者，請務必將旅遊史主動告知學校，以利學校安排健康管理相關措施，如因隱匿而造成校園防疫破口，將依校內獎懲規定追究其責任；於出國期間，倘與確診病例有所接觸者，返臺入境時應主動誠實申報，並通報1922及學校。
二、自109年3月起從境外返臺之教職員工生（包括境外生），請務必配合填寫【境外返臺之教職員工生自主通報】https://www.ndhu.edu.tw/p/423-1000-2949.php?Lang=zh-tw
三、確認須居家檢疫者，請確實遵守，擅離居家檢疫場所會被開罰。
四、確認須自主健康管理者，請配合如下防疫措施：
  (一)依指揮中心「具感染風險民眾追蹤管理機制」規定，應儘量避免出入公共場所，請  
     在家或宿舍休息，不要到校上課。
  (二)在家或宿舍休息之學生，不列入缺課紀錄，並請利用本校安心就學方案，安排學習
     銜接。
五、敬請返臺入境時應主動誠實申報學校防疫專網【境外返臺之教職員工生自主通報】。

{Emergency Epidemic Prevention Notice} All Personnel Returning from Overseas Should Complete Self-Reporting

Dear faculty, staff, and students,

In response to the rapid rise in the global epidemic situation, the Ministry of Education has notified all universities on 2020/03/16 to implement health management for all personnel returning from overseas.

THE UNIVERSITY cooperates with health management measures as follows:
I. All personnel who returned from personal trips should report the travel history to the University for arranging health management related measures.　All personnel should honestly report the contact history with any confirmed case during the trip at the customs on entering the port, and report to 1992 as well as the University afterwards. 

2. Starting 2020/03, all personnel returning Taiwan from overseas (including overseas students) should fill out "Self-report by Personnel returning to Taiwan from overseas" https://www.ndhu.edu.tw/p/423-1000-2950.php?Lang=zh-tw 

THE UNIVERSITY will initiatively contact by phone based on the self-reporting system to determine whether home quarantine or self-health management is needed.
3. Anyone required to have home quarantine should cooperate with the following:
(1) For international teachers and students, the University will arrange quarantine dormitories.
(2) Local personnel should arrange teaching, administration, study, etc., and avoid returning to 
school.
(3) The University will provide health education, and reminds those leaving home without permissions of necessary penalties.
4. Personnel verified for self-health management should cooperate with the following:
(1) According to the "Tracking and Management for People at Risk of Infection" of the Epidemic Command Center, they should avoid going to public places as much as possible, but rest at home or dormitory.  Avoid going to school.
(2) Students who rest at home or in the dormitory should not be recorded as skipping class. They can contact instructor for alternative studies.
5. All personnel returning Taiwan from overseas (including overseas students) should fill out "Self-report by Personnel returning to Taiwan from overseas" right after entering Taiwan.
