標題： 【重要通知】學生團體保險理賠申請注意事項－公告 【Notification】Application of student group insurance claim

親愛的同學大家好：
即日起，學生團體保險理賠請至【生活輔導組】辦理申請，申請注意事項如下：
一、申請收件為週一至週五早上9時至下午4時，地點位於行政大樓生活輔導組辦公室#1櫃台。
二、每週四下午2時至4時有保險公司派駐人員到校服務，如有相關理賠申請問題請適時於派駐時間提問。
三、提出申請期限：無論事故傷害或疾病住院理賠申請，請於1個月內提出；如療程未結束請提出申請延期最長不得超過兩年請求權。
四、應繳交文件：
    1.學生團體保險金申請書(未滿20歲,法定代理人需簽名)
     及個資同意書。
    2.就診收據（正本或副本蓋有與正本相符之戳章）。
    3.診斷證明書(若不同醫療院所各一份,註明所有就診日期)。
4.郵局或銀行存摺封面影本1份。

Student group insurance claims are handled by the Student Living Services Division, as of today.
1.	Service hours: 9:00-16:00, Monday through Friday at Counter #1
2.	Consulting hours: 14:00-16:00 on Thursdays by the staff of insurance company
3.	Application deadline: within one month after an accident or injury; could be extended to maximal 2 years via application
4.	Required documents: 
1)	Application form
2)	Personal Information Agreement
3)	Medical receipt(s)
4)	Certificate of diagnosis
5)	Photocopy of bank passbook
6)	ROC(Taiwan)Exit & Entry Permit or ROC(Taiwan)Resident certificate
